Can you help me to see the difference in usage for the two phrases, "the same" and
"same"?
I almost always see the "the" accompanying the word "same" as it is being used in
writings. Do you always have to place the "the" w/ that word? I think not, but can you give
me some good examples?

Yes. It should be preceded by 'the'.
However, in informal spoken English it often isn't.
eg He passed the exam, same as me.
